UWLEAH Fellowship Application Form

Use this form to begin your application for the UW LEAH program. When instructed on the form, please provide the following: 

  • A brief personal statement describing your background, particular interests in adolescent health, and how LEAH multidisciplinary training will further your goals
  • Curriculum Vitae or Resume

UW LEAH Long-Term Fellow applicants should submit 2 letters of recommendation via email to the UW LEAH Program Manager at uwleah@seattlechildrens.org. 

Please note, that letters are not required for Certificate Trainee applicants including those from Heritage University.
